OSHA Fines a Metro Chicago Manufacturer Failed to Train Workers in Lockout/Tagout Procedures
Chicagoland – A follow-up workplace safety inspection at a manufacturing facility in metro Chicago revealed that the company continues to fall short of its responsibility to protect workers by adequately training them in machine safety procedures.
The U.S. Department of Labor’s Occupational Safety and Health Administration conducted the inspection at a Chicagoland Facility in January 2024 to ensure compliance after previous inspections in October and December 2022 identified workers being exposed to machine hazards.
OSHA proposed penalties totaling $145,415 after citing the company for one repeat violation and four serious violations, including:
- Failure to provide training to authorized employees involved in service and maintenance tasks, such as mold changes on production lines.
- Not adhering to machine-specific energy control procedures during a mold change.
- Exposing workers to an unguarded chain and sprocket on a crusher machine.
- Allowing employees to operate forklifts without the necessary training.
- Exposing workers to trip hazards due to floor panels in poor repair.
“Disregarding OSHA and industry-recommended machine safety procedures is a leading cause of injuries in the manufacturing sector,” stated OSHA Chicago South Area Director. “Workers must receive training in the specific safety procedures for each machine they operate or service, and they should never be exposed to operating machine parts. Employers bear the responsibility of ensuring safe working conditions for all employees within their facilities.”
OSHA offers resources on lockout/tagout and other machine safety procedures to help employers understand how to safeguard workers from potential safety and health risks.
The company has 15 business days from receiving the citations and penalties to comply, request an informal conference with OSHA’s area director, or contest the findings before the independent Occupational Safety and Health Review Commission.
